The Division of Public Health’s Bureau of Oral Health Dental Services (BOHDS) will help you and/or your child achieve the smiles you deserve.  Dental screening, referral, and treatment services are available to help prevent dental infections, decay, and pain.

• A CHECK NEEDS TO BE IN THE BOX TO ALLOW US TO CORRESPOND WITH YOU
• IS THE PATIENT RESPONSIBLE FOR THE BILL:  Screen defaults to YES, do not change. You will not receive a bill for services provided. 
• CHECK BOX TO PROVIDE CELL PHONE NUMBER:  Allows us to send Information about dental services received; have a member of the dental team respond to your questions; and provide you with resources and information at your request.
