The Division of Public Health’s Bureau of Oral Health Dental Services (BOHDS) will help you and/or your child achieve the smiles you deserve. Dental screening, referral, and treatment services are available to help identify dental infections, decay, and pain.

"IMPORTANT TIPS FOR COMPLETING REGISTERATION- WHAT TO DO WHEN I SEE THIS SCREEN" 

1. SIGNATURES: All forms need to be signed with the legal guardians signature unless you are over 18 and the patient will sign their name.
2. RED * Only fill out the boxes with the red *
3. IS THE PATIENT RESPONSIBLE FOR THE BILL:  Screen defaults to YES, do not change. You will not receive a bill for services provided. 
5. CHECK BOX TO CORRESPOND:  Allows us to send Information about dental services received; have a member of the dental team respond to your questions; and provide you with resources and information at your request.
